The following is a description of the FREE program we offer through our Non Profit Organization: We are a non-profit organization that promotes wellness and independence by providing low-cost transitional housing for vulnerable persons, such as individuals with mental health and/or addiction problems. The transitional housing is coupled with services that offer structure, support and supervision to help occupants gain or regain valuable and meaningful roles in the community. The Organization realizes that many individuals who reside outside our houses can also benefit from these free services, hence the creation of the Mental Health Outreach Program. The Mental Health Outreach Program is a free and holistic program based on the belief that individuals must embrace change in many areas of their lives in order to experience positive outcomes, and that single interventions in isolation are not as likely to be effective. The key goals of the program look to reduce mental health symptoms, increase housing stability, heighten independence, greater life satisfaction, improve employment and educational outcomes and enhance social and family relationships. The Organization offers the Mental Health Outreach Program as an additional service for occupants and other eligible members of the public. The Mental Health Outreach Program is only available to individuals who satisfy specific inclusion criteria; who are looking to live a safer, healthier and more simplified life; and who share the Organization’s belief and commitment to holistic change. Each participant is assigned a support coordinator to assist in the creation and implementation of an action plan. The Organization’s services, referrals and support will assist the participant to reach their specific goal(s). Progress updates, medical appointment compliance, medication compliance, counseling and random drug testing may be required for program maintenance. Mandatory services are assessed on an individual case basis. Free services included: - case management - group counseling and individual counseling including psychotherapy - food vouchers - medication monitoring -transportation services (bus passes and taxi) -community reintegration assistance We would be happy to discuss any further questions you have.
